Happy Holidays SLA-ers!

As a gift of appreciation and to celebrate the holidays the SLA is giving each of our employees $250 in gift cards.

Below you will select five $50 gift cards totaling $250. You can select five different cards, five of the same card, or any combination of gift cards you want from the selections below.

You will need to put your FIRST and LAST name, SLA EMAIL ADDRESS and EMPLOYEE ID NUMBER in the form below to receive your e-cards.

You can find your EMPLOYEE ID NUMBER in Paylocity. Log into Paylocity (click here to open Paylocity in a new browser window) and hover over your picture or avatar icon in the upper right corner of the screen. The number in brackets [] next to your username is your employee ID number.
